Churchwell, Sr.; his wife Clara Faylene Churchwell; his children, James Curtis Churchwell, Jr. (Kimberly Rivera) and Angela Churchwell Shealy; his grandchildren, Justis Brady Churchwell, Presley Ann Churchwell and Autumn Cheyenne Churchwell Shealy; his great grandchild Carter Dallas Churchwell; his siblings, Emma Mae Walters and Robert Churchwell; and his nephews, Donald "Sonny" Churchwell, Jr. and Steven Ford. He is also survived by many other brothers, sisters, and loving family members.
