He was predeceased by : his wife Carol Cook; his parents, Louis Thomas "Tom" and Pearl Austell Cook; and his siblings, Ima Lee Cook, Dwight Franklin Cook and Gilbert Cook.

He is survived by : his children, Janet Leah Thomas (Jim), Shelia Slatton (George) and Bart Cook (Anne); and his siblings, Marriel Humphrey (Leon), Mara Nell Bradley (John), Wallace Cook (Dean), Earl Cook (Joan) and Louis Cook (Martha). He is also survived by other nieces, nephews, cousins, other family members and friends.

A reception was held on Wednesday, May 1st 2024 from 6:00 PM to 8:00 PM at the Moss Service Funeral Home (1901 2nd Ave NW, Cullman, AL 35055). A burial was held on Thursday, May 2nd 2024 at 2:00 PM at the Oakwood Cemetery.
