He was predeceased by: his wife Mary Ida Maddox; his former wife Mary Frances Maddox; his daughter Carolyn Nichols; his parents, O. W. Maddox and Joanna Maddox; and his siblings, James "Red" Maddox, Joey Maddox, Quinton Maddox, Ellie Pigg, Anna Belle Johnson, Opal Blackwood and Eva Laursen.
He is survived by: his granddaughter Kimberly Pratt; his great grandchild Samara Pratt; and his sisters, Cora Pigg and Betty Adams. He is also survived by nieces and nephews.
