He was predeceased by: his parents, Ruby Hayes Perkins and Clyde Perkins; his brothers, Charles Perkins, Rudolph Perkins, Frank Perkins and Chester Perkins; his son Bret Perkins; and his grandson Cayn Perkins.
He is survived by: his wife Sharon Kay Perkins; his children, Jerrilyn Perkins, Hilyre Hayes Baker (Nathan), Jami Nicol Perkins, Jerry Perkins II (Kayla) and Carter Hugh Perkins; and his grandchildren, Ruby Hayes Baker, William Everett Baker, Penelope Grace Box and Jerry Perkins III. He is also survived by brothers in law, sisters in law, nieces and nephews.
