He is survived by: his significant other Joy Tyson; his mother Mary Ann Boggs; his sons, Jason Eubanks and Phillip Eubanks; his granddaughter Sela Eubanks; his daughter-in-law Kristina Haines; and his cousins, Warren, Nikki Clark, Mark, Jewel Thomas, Bobby Sizemore, Linda Sizemore and Joanne Sizemore.
