He was predeceased by: his wife Hazel Adams Mathews; his parents, T. J. Mathews and Matilda Heath Mathews; and his brother Thomas Mathews. He is survived by: his children, David Mathews, Jr. (Vanesa), Adam Mathews (Valerie) and Betsy Peacock (John); his grandchildren, Joshua Mathews, Victoria Mathews, Maci Mathews, Trace Mathews, William Peacock and Ethan Peacock; and his brothers, Donald Mathews, Dale Mathews (Brenda) and Larry Mathews (Joann).
