He was predeceased by: his parents, Andrew Jackson Church and Bessie Elizabeth Hoyle Church; and his brothers, Billy Church and Bobby Church. He is survived by: his wife Betty J. Church; his children, Winona Church Glasscock (Jamie), Kellie Parker (Michael), Jackie David Church, II (Dianna), Steven Marcus Church (Angelina) and Morgan Sims; his sisters, Mary Nell Martin and Tommie Lou Miller; and his close friends, Ronnie Maske, Billy Wayne Mann and Paul Bishop. He is also survived by twelve grandchildren; nine great grandchildren.
