He was predeceased by: his father James E. Self; his stepmother Frankie V. Self; his mother Helene Cataldi; his stepfather Rudi Cataldi; and his sister Gloria Alford.
He is survived by: his wife Elizabeth Self (Libby); his brother Tommy Self; his children, Richard Self (Cindy) and Dena Cook (Jon); and his grandchildren, Bailey Gunter (Graham), Jon Cannon Cook, Tyler Self (Katey) and Brodie Self. He is also survived by several nieces and nephews, and some very special friends.
