He was predeceased by: his parents, Einar Mikkelsen and Ruth Mikkelsen; and his sons, Dustin Mikkelsen and Dennis Clark. He is survived by: his wife Sarah Mikkelsen; his daughters, Valerie Mikkelsen Montgomery (Todd), Tricia Mikkelsen, Denise Clark and Denessa Chancy; his grandchildren, Timothy Raughton, Thomas Roughton, Hannah Chandler (Adam), Catherine Mikkelsen (Jacob), Peyton Cunningham and Micah Montgomery; his great grandchildren, Hunter and Laine Lochrico; and his sisters, Annette Blaylock (Don) and Dr. Carol Mikkelsen.
