He was predeceased by: his father Oscar Fortis; his former wife Dixie Fortis; and his siblings, Nicholas Fortis and Lenore Fortis. He is survived by: his wife Claudia Hammer Fortis; his stepson Robert C. Mellon (Christy); his children, Cherie Fortis and Callin Scot Fortis (Suzette Kealen) of Miami; his grandson Avery Ryder; his great grandchildren, Atlas Reed and Teo Fiel of Boulder; his sister Jodi; his nephews, Bill Fortis and Paul Fortis; and his cousin Diane Hawthorne.
In lieu of flowers, donations may be made in his name to Jerry's favorite non-profit cat rescue organization, The Cat Coalition in Fairhope, AL. (Attn. Glenda Turner).
