He was predeceased by: his son Chad Horn; his parents, Alvin Horn and Flora Horn; and his siblings, Margaret Faye Sala, Shirley Hardaman, Ken Horn and Leslie Horn.
He is survived by: his wife Carol Lee Horn; his children, Scott Horn (Donna) and Amy Conn (Alan); his grandchildren, Kalyne Roberts (Tyler), Dawson Horn, Kaylei Horn and Jacob Conn (Caroline); his great grandchild Bennett Conn; his siblings, Carolyn Burroughs, Nicole Waters, Kim Ingram, Mike Horn and Barry Horn; and his pet Lili Bet. He is also survived by numerous nieces and nephews.
