She was predeceased by: her parents, Hilton Cook and Pheobia Cook; and her brother Keith Cook. She is survived by: her brother Kenny; her son Chase Clark (Amber); her grandchild Haiston Whitehead; her sister Linda Fisher; her sister-in-law Teresa Cook; her nephews, Scott Anners (Dana), Brad Trujillo and Briceson Cook (Olivia); her great-nieces and great-nephews, Maddie, Andrew and Will Anners; and her caregivers, Erica Helms, Kylie Toscano, Hannah Outlaw, Cassidy Austin and Krisie Arnold. She is also survived by her father, her son, and her entire family.
