He was predeceased by: his parents, Buck Cook and Katie Bell Cook; and his siblings, Barbara Wright and J. W. Smith. He is survived by: his children, Brandy Ward (Mike Smith), Amy Cook and Matthew Cook (Shannon); his brother Rickey Cook; and his great grandchild Addisson. He is also survived by numerous nieces, nephews, and cousins.
