He is survived by: his wife Wilda Whisenant Hendrix; his daughters, Milla Wetzbarger (Mark), Kam Acupan (Eugene) and Jana Deason (Chris); his grandchildren, Lauren Grace Acupan, Philip Deason and Matthew Hendrix; and his brother Larry Hendrix (Deborah).

In lieu of flowers, the family kindly requests donations be made to St. Jude Children's Research Hospital In Memory of Mr. Phil Hendrix.
