He was predeceased by: his parents, Jesse C. Cook and Violet E. Judy Cook; and his siblings, J. C. Cook and Rose Marie Chance. He is survived by: his sons, Brandon and Adam Cook; and his siblings, Bill Cook, Betty Shepherd (Victor) and Mary Oates. He is also survived by nieces and nephews.
