He is survived by: his wife Katie; his sons, Jerry Jr. (Cindy) and Nick (Jill); his grandchildren, Kyle Dunlap, Craig Lewis, Blake Lewis (Carlee), Jillian (Matt), Jessica Beard (Shelton) and Emily Lewis; his great grandchildren, Charley, Drew, Harrison, Leandor, Tucker Rex, Scarlett and Sophia; and his close friend Wayne Brown.
