She was predeceased by: her close friend Wendel Collins; her parents, Rosa Lee and Samuel Fred Forman; and her brothers, Robert, Sammy, Jerry, Ronald Dale (Daleno), Jimmy and Johnny Forman.
She is survived by: her children, Paula Kelley, Johnny Craig and Jeff Cunningham; her grandchildren, Nikki Kelley, Jared Kelley and Samantha Warren; her great grandchildren, Kaitlyn Duffey, Natalie McGraw, Jackson and Joseph Kelley; and her brother Gary Forman. She is also survived by nieces, nephews and extended family.
