He was predeceased by: his parents, Lander Forman and Gladys Forman; his wife Margaret La'nell Forman; and his siblings, Denson Forman, Kenneth Forman and Sandra Denney.
He is survived by: his daughter Scarlet Alexander of Munford; his grandchildren, Joshua Alexander and Brooklyn Young; and his sisters, Jillian Estes of Moody and Deborah Forman of Anniston. He is also survived by several nieces, nephews and a large extended family and many friends.
