He was predeceased by: his parents, John Calvin Blake, Sr. and Donnie Elizabeth Fuller Blake; and his siblings, Alice Blake and James Blake. He is survived by: his significant other Carmen Forbus Blake; his son Scott; his daughter-in-law Cindy Blake of Alexander City, Alabama; his daughter Karla; his son-in-law Kelly Burdette of Killen, Alabama; and his grandchildren, Lynden Blake (Chad Balyo), Michael Blake, Weston Blake, Blakely Burdette and Joseph Burdette.
In lieu of flowers, memorials may be made to, First Baptist Church Alexander City.
