She is survived by: her husband Charles R. Smith; her children, Wanda Weldon (Wade) and Randy Smith (Cathy); her grandchildren, Justin Weldon (Kayla), Ashley Carter (Stevan), Mitchell Smith, Heather Smith, Amber Davis and Marley Mann; her great grandchildren, Grace Sweat, Cade Weldon, Allene Sweat, Aliah Weldon, Waylon Weldon, Alison Carter, Kyndal Davis, Gage Davis and Lenlee Davis; and her brothers, Cecil Formby (Dee) and Lash L. Formby (Martha). She is also survived by nieces and nephews.
In lieu of flowers, the family respectfully requests contributions be made to the family of Gwendolyn Smith in care of Wanda Weldon for burial expenses.
