She was predeceased by: her parents, Robert C. Phillips and Vergie Garner Phillips; her former husband Carel Max Smith, Sr.; her husband Byron M. Sullivan; and her son James Dale Smith.
She is survived by: her children, Max Smith (Carolyn) of Tuscaloosa, AL, Robert Smith (Ellen) of Rockford, AL, John Smith of Rockford, AL and Mary Land (Bobby) of Jay, FL.; and her siblings, Edward London (Lorine) of Sylacauga, AL and Mary Reynolds (Lewis) of Rockford, AL. She is also survived by 11 grandchildren and 16 great grandchildren.
In lieu of flowers donations can be made to the Youth Department at Farm Hill United Methodist Church.
