He was predeceased by: his parents, J. W. Formby and LaRue Formby; his son Blake Formby; and his brother Bar Formby. He is survived by: his wife Mary Beth Formby; his children, Mary Blaine Formby and Dee Gunter Formby (Erica); his grandsons, Joe Formby and Sam Formby; his brother Jim Formby (Tammy); and his sister-in-law Janet Formby. He is also survived by nieces and nephews.
Visitation was held on Tuesday, January 28th 2025 from 4:30 PM to 7:00 PM at the Adams Brown Service Funeral Home (6906 US-431, Albertville, AL 35950). A funeral service was held on Wednesday, January 29th 2025 at 2:00 PM at the same location.
