He was predeceased by: his parents, Henry Adolph Bolz and Johanna Jurgen Dykman Bolz; and his former wife Bonita Hazlett. He is survived by: his wife Phyllis White; and his children, Jerry White, Karen Litcher, Cyndi Bolz, Debbie White and William Bolz. He is also survived by numerous grandchildren, great-grandchildren and great-great-grandchildren.
