He was predeceased by: his parents, Gail E. Chezik (Pasold) and Francis William Chezik (Bill). He is survived by: his sons, Christopher William of Soldotna, Matthew John of Soldotna and Jeremy Michael of Soldotna; his siblings, Nick Chezik (Leanne) of Blue River, WI, Ted Chezik (Judy) of Wisconsin Dells, WI, Ava Eubanks of Prarie, Amy O'Herrin (Patrick) of Friendship, WI, Ella Chezik of Soldotna, AK and Lisa Johns; and his grandchildren, Keelin and Connor of Soldotna. He is also survived by 4 nieces and 6 nephews.
