He was predeceased by: his father Glen October21; and his grandparents, Homer, Elsie Chase, Peter and Francis Miszczak.

He is survived by: his mother Elizabeth "Chris" Chase October21; his daughter Hailey (Joey); his close friend Coralee Eytcheson; his children, Christina Chase (Colton Karst) and Nathan Chase; his stepson Cameron (Christine Eytcheson); and his granddaughters, Remedy, Jacqueline and Elouise.
