He was predeceased by: his parents, Matt Petlak and Olga Petlak. He is survived by: his wife Diana Petlak of Peter's Hospital; his sons, Stuart Petlak and Jason Petlak; his grandsons, Kain Petlak and Logan Petlak (Kristin); his great grandchildren, Kohen Petlak and Kesler Petlak; and his brother Don Petlak (Carol).
