He was predeceased by: his grandchild Montana; and his parents, Gerardus and Christina. He is survived by: his wife Theresa; his children, Lori Martinez (Lee), Angela Haverkate (Corey Ivany), Tracey Kury (Rick) and Christine Bulishak (Ben); his grandchildren, Taylor (Jacob), Britney, Sheila, Autumn (Ryan), Paris and Dakota; his great grandchildren, Arianna, James, Connor, Harper, Everly and Sarah; and his sister Christine Byloo. He is also survived by his many nieces, nephews, extended family, and friends.
