He was predeceased by: his wife Deborah; his parents, Audrey Cook and Gerald Cook; and his siblings, Clair (Kaye) and Darlene (late Mark).

He is survived by: his children, Gene (Nicole) and Jeff (Kenya); his grandchildren, Deborah, Cheyenne (Jeremy), John, Logan, Natalie (RJ), Rhiley and Reese; his siblings, Marilyn (late F. E.), Marvin, Allan (Lila), Jerry, Cliff, Corey, Kim and April; and his friend Tom. He is also survived by many nieces, nephews.
