He is survived by: his wife Kathryn; his mother Martha; his siblings, Margaret Ann, Douglas and David; his children, AJ (Cara), Grant (Paula) and Amy (Dan); and his grandchildren, Makenah, Morgan, Drew, Charlotte, Connor, Alexis and Hunter.

In lieu of flowers Jack would appreciate donations be made in his name to the Terry Fox Foundation for Cancer Research or to Blue Springs Scout Reserve "add 100" in memory of Jack.
