He was predeceased by: his wife Lucille. He is survived by: his daughter Bonnie Kozak (Tim); his grandchildren, Ryan (Deb) and Marissa (Farley); his daughter Cindy Langen; his grandchildren, Jesse (Julia), Justin (Julia) and Tessa; his son Tom (Jackie); his grandchildren, Rob, Lyndsay (Rajan) and Andrew; and his siblings, Trudy (Ed), Sandra, Linda, Tim (Norma) and Eugene (Pat). He is also survived by nine great-grandchildren.

In lieu of flowers, donations may be made in Tom's memory to Ronald McDonald House.
