He was predeceased by: his parents, Josephine Kowalchuk and Steven Kowalchuk; his siblings, Joe Cherminski (Gertrude), Wally Cherminski (Marie) and Veronica Yarzab (Stan); his nephew Ronald Yarzab; his daughter-in-law Anna Pucci-Kowalchuk; his parent-in-law Biagio Campagnano; and also Antoinette Campagnano.

He is survived by: his wife Carmel; his sons, James (Nicole), John (Diane) and Rob (Sameral); his grandchildren, Lindsay Madore (Kyle), Taylor Kowalchuk (Tom Schuster), Jamie Kowalchuk (Lee Risi), Robyn Kowalchuk and Ryan Kowalchuk; and his great grandchildren, Scarlett Madore and Lennox Madore.

If you so desire, donations may be made in Eugene's memory to the Alzheimer's Society, The Canadian Diabetes Association, St. Joseph's Care Group or a charity of your choice.
