He was predeceased by: his brothers, Harry (Frances), John (Elizabeth) and Ralph. He is survived by: his wife Annie Kok (Nee Keizer); his children, Debbra Schalk (Eric), Jerry Kok (Freda), Heather Wright (Barry) and John Kok; his grandchildren, Michael (Amanda), Nicole (Kevin), Colin, Cassandra, Emily (Joseph), Lauren and John; his great grandchild Hudson; his siblings, Ron (Hilda), Wendy and Jerry (Barb); and his sister-in-law Anne. He is also survived by his many nieces, nephews and extended family.
