She was predeceased by: her husband Bob Kozak; her son Rob Kozak; her grandchildren, Godwin and Gregory; and her siblings, Don, Evelyn and Shirley.
She is survived by: her child Kim Kozak; and her sister Donna Milmine. She is also survived by many nieces, nephews, cousins and friends.
