He was predeceased by: his parents, Steven Kozak and Sophie Kozak; and his brother Fred. He is survived by: his wife Ann; his daughter Sandra; his stepdaughters, Debbie and Annette; and his grandchildren, Aleksei, Shawn (Shea), Joshua (Taylor), Nick (Michelle), Tyler (Dana) and Jim (Andrea).
