He was predeceased by: his father Sheldon Church. He is survived by: his children, Jordan, Josh and Caitlin (Andrew Korschefsky); his parent Christena Church; and his siblings, Calvin, Evans (Michelle) and Kelly (Jodi). He is also survived by many friends, aunts, uncles and cousins.
