He was predeceased by: his wife Carol. He is survived by: his children, Stuart Cook, John Cook (Pat), Kate Pattison (Robert) and David Cook (Carol); his grandsons, Christopher, James and Matthew; and his great grandchildren, Lyanna and Ivy.
