Hazlewood was predeceased by: spouse Linda; and parent Audrey. Hazlewood is survived by: children, Jonah (Kat), Aaron, Jeff Mackie, John Farkas (Tessa), Caroline Farkas, Grandpa Doug, Alan, Hayden and Charlotte; parent John T. Hazlewood (Lynn); and siblings, Don Hotson (Angela) and Debbie Hotson (Geoff). Hazlewood is also survived by several nieces and nephews.

In lieu of flowers donations to Paris Museum or Heart and Stroke Foundation would be appreciated.
