He is survived by: his daughter Roxanne Cook (Debbie Cook); and his grandchildren, Harvey Cook, Landon Cook, Damon Cook, Chris Cook, Chloe Cook, Melissa Cook, Morgan Cook and Montana Campbell Cook.
