He was predeceased by: his wife Barbara Ann Evestaff; his parents, Stephen Jones and Mary Smith; and his sister-in-law Debbie. He is survived by: his children, Tammy, Kevin (Joyce), Jennifer and Daniel (Sugar); his siblings, Stephen (Dilys), Carol and Trevor (Sylvie); and his siblings-in-law, Frank (Evestaff), Deb (Khoury), Susan (Evestaff), Steven (Hughes), Gilbert (Borgogno), Joanne and Adam (Salim).

In lieu of flowers, donations in David's memory can be made to the Canadian Diabetes Association and/or to the Kidney Foundation of Canada.
