He was predeceased by: his brother Ota. He is survived by: his wife Shirley; his children, Henry Bartak and Carmen Bartak (Steve); his stepson Stephen Jackson (Amy); his grandchildren, Hayden and Lucas; his sister Lida Sipla; and his nephews, Vaclav and Dominik Sipla of Boston, MS.
He fought a brave battle with Parkinson's for sixteen years and Dementia for three years.
