He is survived by: his spouse Jen Lake of Keswick; his children, Stacey Lee (Bob Cowie) of Peterborough, Tim (Terra), Michaela (Nick), Tiffany (Steve Roorda) of Barrie, Alexis (Dylan Thompson) of Lindsay and Lauren Chadwick of Keswick; his grandchildren, Emily, Cole, Audrey and Hennessey; and his sibling Ricci Lake of Midland. He is also survived by several nieces, nephews, relatives and friends.

In memory of Bert, donations to the Neuroscience Unit of St-Michael's Hospital, Toronto, would be appreciated.
