She was predeceased by: her parents, Walter Smith and Annie Smith of Ladysmith, QC; her husband Robert of Kemptville; and her siblings, Herbie, Mervin, Vertal, Harper, Della Palmer, Velma Holt and Lois Smith.
She is survived by: her children, Fay McNeilly (Doug) of Oxford Mills, Glen (Barb) of Portland, Ron (Merna) of Smiths Falls and Heather Paveley (Chris) of Renfrew; her grandchildren, Amy McNeilly (Jeff) of Smiths Falls, Angela Kennedy (Stacy) of Thompson, MB, Sara McNeilly of Brockville, Allen (Melissa) of Ottawa, Matthew Paveley (Angie) of Renfrew, Dawn of Portland and Kandyce Campbell (Cory) of Toronto; and her great grandchildren, Taylor, Chase, Dakota, Jase, Brody, Gavin and Alaya. She is also survived by many nieces, nephews and friends.
In lieu of flowers, donations may be made in Hazel's name to the Kemptville District Hospital Foundation.
