He was loved and cherished by many people including: his girlfriend Liz; his children, Chris (Ashley), Mike, Courtney (Brandon), Dana (Scott) and Christopher; his grandchildren, Edie, Alice, Keilan, Drey, Zayn, Lennon, Knox and Ada; his parents, Viola Cook and Bruce Cook; his siblings, Brenda (Dan), Marlene (Ron) and Allan; and his caregivers, Betty, Carol, Meghan and Tracy. He was also cherished by his many nieces, nephews, and friends.

As an expression of sympathy, donations can be made in David's name to the Haliburton Highlands Health Services Foundation would be greatly appreciated by the family.
