He was predeceased by: his sisters, Myrtle, Eileen, Betty and Barbara Jean; and his parents, Herb Howlett and Olive Howlett.
He is survived by: his wife Linda Scott; his children, Ken, Vicky of Fergus, Mark and Cindy of Fergus; his stepchildren, Melanie, Chris of Toronto, Jessica, Jamie of Belwood, Johanna and Max of Fergus; and his grandchildren, Finley, Maisie, Isobel, Elijah, Charlotte and Lincoln.
Memorial donation in Jake's memory may be made to Groves Memorial Hospital.
