He was predeceased by: his spouse Monique; his parents, Dominica Kazulak and Arthur Kazulak; and his sisters, Lois, Helen and Sylvia.
He is survived by: his children, Mark of Hamilton and Perry (Heidi) of Youngstown, NY.. He is also survived by many nieces, nephews and numerous friends.
