He was predeceased by: his parents, Kenneth Raymond and Audrey May Cook. He is survived by: his wife Susan Cook; his children, Cailtin Cook (Melissa Dover) and Natalie Liebno (Darrell Liebno); his grandchildren, Greyson Liebno and Chase Liebno; and his siblings, Dan Cook, Kim Cook and Ron Cook.
