He is survived by: his spouse Pam Blakley; his children, Nicole, Cody, Mathew, James and Hannah; his grandchildren, Hailey and Dominic; his parents, Wayne Monk and Delrene Monk; his brother David; his parents-in-law, Jim Blakley and Donna Blakley; his brother-in-law Ron Blakley; and his nieces, Ava and Morgan.
