He was predeceased by: his grandfathers, Edward Kwasyncia (Doris) and Alphonse (Isabelle Martens). He is survived by: his parents, Krisan Sereda (Fred Feenstra) and Stephen Martens (Cecelia); his children, Ella Rose, Samantha, Veronica and Claire Poolman; his brother Brandon; his stepbrothers, Jessie and Zachary Martens; his significant other Sarah Nicol; and his grandparents, Thomas (Betty Braekevelt) and Germaine (John Tulen). He is also survived by many aunts, uncles and cousins.
In memory of Chad, donations may be made to an Education Fund for Ella Rose (cheques only, payable to Krisan Sereda).
