He was predeceased by: his daughter Kayla. He is survived by: his girlfriend Tammy; his daughter Tara; his stepchildren, Stephanie (Curtis), Matthew and Spencer (Crystal); his parents, Donna and Ken; and his siblings, Sarah and Jeff.
