He is survived by: his wife Shelly Forestell; his children, Tash Assinck, Devin Schaefer and Mitch Scott; his granddaughter Mackenzie; his siblings, Harold Forestell, Agnes Zukowsky, James "Ted" Forestell (Cynthia), Nancy Kingsley and Rose Wickman; his mother-in-law Diane Holmes; his brother William Forestell; his brother-in-law Ray Wickman; his father-in-law Larry Holmes; and his parents, Helen Forestell and Donald Forestell. He is also survived by nieces and nephews.
Donations to the Alzheimer's Society which was close to Rick's heart and would be appreciated by the family.
